Preliminary findings of a recent railway accident that resulted in over 10 fatalities and approximately 50 injuries indicate human error as the cause. It was revealed that the Visakhapatnam-Rayagada train overshot a red signal, leading to a collision with the stationary Visakhapatnam-Palasa train.
